2. Complete the information requested in the referral form. –> Select your name from the list below*
* Please notify your branch manager if you do not see your name as an option for your branch.Please fill out as much information as possible.
3. Under Attach Merchant Services Statement, click the ‘File to Upload’ button, then attach the customer’s current merchant services statements (these are the statements from their existing merchant services provider. If the customer is not currently accepting credit cards, include that information in the notes section of the referral form).
